This article will guide you to locate the Digital Health Screen Questionnaire via the Member App and where to find the members completed questionnaire on their Exerp profile.
Where to locate it on the Member App
- Members will log into the Member App and navigate to My Membership.
- Members will complete the questionnaire by clicking on the button Health Screen Questionnaire.
Members will be required to sign in with their details to reach the form:
How to check on Exerp
- On the members profile, navigate to Journal > Questionnaires.
- Ensure that you change the state to 'all' to be able to view their DHSQ form and adjust the dates if needed. Make sure to check the status is completed.
